An unusual case of dramatic acute bilateral pyelonephritis with systemic bacterial dissemination caused by uropathogenic Escherichia coli.

چکیده
Pyelonephritis is the most severe form of urinary tract infection (UTI) and occurs more frequently in women than in men [1]. Escherichia coli is the pathogen most often involved and causes >80% of communityacquired UTIs as well as 50% of UTIs in hospital patients [2]. Although antibiotic treatment is effective in most cases, recurrent kidney infection may lead to chronic pyelonephritis in some patients. Here, we report a case of acute bilateral pyelonephritis due to an E.coli isolate which, despite antibiotic therapy, led to systemic bacterial dissemination and very rapid destruction of both kidneys. This infection was associated with prolonged septic shock that unavoidably led to bilateral nephrectomy. Host defences and pathogen virulence were studied.

Comparison of Genetic Patterns of E. coli Strains Isolated from Patients with Cystitis and Pyelonephritis, Using Pulsed Field Gel Electrophoresis
Background & Aims: Urinary tract infection (UTI) is one of the most frequently acquired bacterial infections caused by a large genetically heterogeneous group of Escherichia coli which are called uropathogenic E. coli (UPEC). Cystitis and pyelonephritis are two most common symptoms seen in patients with UTI. The genetic diversity of this organism has hampered the identification of UTI strains a...
متن کاملC5aR1 promotes acute pyelonephritis induced by uropathogenic E. coli
C5a receptor 1 (C5aR1) is a G protein-coupled receptor for C5a and also an N-linked glycosylated protein. In addition to myeloid cells, C5aR1 is expressed on epithelial cells. In this study, we examined the role of C5aR1 in bacterial adhesion/colonization of renal tubular epithelium and addressed the underlying mechanisms of this role. We show that acute kidney infection was significantly reduc...
متن کاملDissemination and Systemic Colonization of Uropathogenic Escherichia coli in a Murine Model of Bacteremia
Infection with uropathogenic Escherichia coli (UPEC), the causative agent of most uncomplicated urinary tract infections, proceeds in an ascending manner and, if left untreated, may result in bacteremia and urosepsis. To examine the fate of UPEC after its entry into the bloodstream, we developed a murine model of sublethal bacteremia. CBA/J mice were inoculated intravenously with 1 × 10(6) CFU ...
متن کاملComplicated bilateral fungal emphysematous pyelonephritis in a 55-year-old diabetic female: A case report and review of literature
Background and Purpose: Emphysematous pyelonephritis (EPN) is a rare and serious disease causing acute renal failure. Diabetes is a major risk factor for this infection. Case report: Herein, we present the case of a 55-year-old female patient with diabetes and EPN caused by Candida albicans. The infection was complicated with endophthalmitis and endocarditis. The results of antifungal suscep...
متن کاملExpression of P fimbriae of uropathogenic Escherichia coli strains.
The occurrence of P fimbriae in a total of 222 uropathogenic Escherichia coli (UPEC) strains was investigated. Out of the total, 31 (14%) were P fimbriated. Of 24 pyelonephritogenic E. coli strains, three (13%) with P fimbriae occurred in children with clinical pyelonephritis, and of 198 E. coli strains 29 (15%) occurred in children with cystitis. Prevalence of P fimbriae of E. coli strains was...
